Complete all necessary documentation online such as health history and consent forms. (You can find these in the "Patient Forms" section of the "Patient Resources" page.)
Fill out the COVID-19 Screening Form, which you will receive via text prior to your appointment.
Text us upon your arrival and wait in the car until a member of the office comes out to get you.
Please bring a face mask to wear within the VDA facilities, if you don't have one we can provide you with one.
Your temperature will be taken upon arrival to verify the absence of fever.
